Karleen Insurance Agency, Inc. and our affiliated companies appreciate the trust you have placed in us when you ask us to protect your assets. Because you trust us with your personal information when you purchase insurance through us, we want you to know that we are committed to protecting your private information. Federal Law has established privacy standards and requires us to provide this summary of our privacy policy.

OUR PRIVACY POLICIES AND PRACTICES

1. Information we collect:

    We collect nonpublic personal information about you from the following sources:
    Information we receive from you on applications or other forms, such as name, address, social security number, assets, income, beneficiaries, mortgagees, leinholders, etc.
    Your transactions with us such as policy coverages, premiums, payment history, claims, etc. Your transactions with our affiliates such as insurance carriers, banks, finance companies, other insurance agencies and agents, outside claims services, physicians, hospitals and other medical personnel, and insurance supported organizations.
    Information we receive from a consumer reporting agency such as, but not limited to, motor vehicle reports, CLUE reports, credit scores and reports, investigative reports, surveys.
2. Information we may disclose to third parties:

    We may disclose all information that we collect about you, as described above.
3. Non affiliated third parties to whom disclosures may be made:

    We may disclose nonpublic personal information about you to the following types of third parties.
    Financial service providers, such as insurance companies, insurance agencies, claims adjusting services, mortgage bankers, banks, finance companies, and other sources necessary to provide insurance products and services for you.
    We may also disclose nonpublic personal information about you to non affiliated third parties as permitted by law.
4. Our practices regarding information confidentiality and security:

    We restrict access to nonpublic personal information about you to those employees, affiliates and third parties who need to know the information in order to provide products or services to you. We maintain physical, electronic, and procedural safeguards to protect your non public personal information.
5. Our policy regarding dispute resolution:

    Any controversy or claim arising out of or relating to our privacy policy, or the breach thereof, shall be settled by arbitration in accordance with the rules of the American Arbitration Association, and judgment upon the award rendered by the arbitrators(s) may be entered in any court having jurisdiction thereof.
6. Reservation of the right to disclose information in unforeseen circumstances:

    In connection with the potential sale or transfer of its interests, Karleen Insurance Agency, Inc. and it's affiliates (if any) reserves the right to sell or transfer you information (including but not limited to your name, address, age, sex, zip code, state and country of residency and other information that you provide through other communications) to a third party entity that (1) concentrates it's business in a similar practice or service; (2) agrees to be Karleen Insurance Agency, Inc.'s successor in interest with regard to the maintenance and protection of the information collected; and (3) agrees to the obligations of this privacy statement.
